Anti-platelet therapy for Acute Coronary Syndrome
نویسنده
چکیده
The term ‘Acute Coronary Syndrome’ (ACS) refers to a range of acute myocardial ischaemic states which include unstable angina, non ST segment elevation myocardial infarction (NSTEMI) and ST segment elevation myocardial infarction (STEMI). Despite significant advances in the diagnosis and management, ACS remains a significant global problem with very high mortality and morbidity. The pathogenetic process central to the initiation of an ACS is disruption/fissuring of an atheromatous plaque and consequent exposure of core constituents such as lipid, smooth muscle, and foam cells, leading to the local generation of thrombin and deposition of fibrin. This in turn promotes platelet aggregation and adhesion and the formation of intracoronary thrombus1.

The benefits of anti-platelet therapy in the treatment of acute coronary syndrome (ACS) as an adjunct to percutaneous coronary intervention (PCI) are well established (1,2). However, the appropriate response to resistance of anti-platelet therapy remains undefined. Because universal standards and universal definitions of resistance have not been established, conventional wisdom discourages rout...
متن کاملPlatelet count and MPV, routinely measured but ignored parameters used in conjunction with the diagnosis of acute coronary syndrome: single study center in Iranian population, 2010
Background: Myocardial infarction is the major cause of morbidity and mortality in industrialized countries. Platelet count and the mean platelet volume (MPV), a simple and reliable indicator of platelet size which correlates with platelet activation, might associate with troponin in acute chest pain. Methods: We analyzed MPV of 851 patients who were admitted to Rasoul-e-Akram Hospital with ac...
متن کاملNovel Strategies in Anti-Platelet Treatment for Coronary Artery Disease
Treatment with oral anti-platelet agents constitutes a cornerstone in the therapy of coronary artery disease. Coronary angioplasty and stent implantation improved the therapy of coronary artery disease and especially the treatment of acute myocardial infarction. Implementation of glycoprotein IIb/IIIa inhibition further advanced anti-platelet therapy as a central component in the treatment of a...
متن کاملSafe removal of epidural catheter--a dilemma, in patients who are started on dual anti platelet therapy postoperatively for acute coronary syndrome--a case report.
Epidural catheter insertion or removal in patients receiving antiplatelet therapy for acute coronary syndrome poses a high risk for epidural hematoma. Though practice guidelines suggest stopping clopidogrel for at least 7 days before such intervention. Withholding anti-platelet drugs for such a long duration represents a great risk to these patients. We present a case of a 53 year old male pati...
متن کاملCurrent practice of percutaneous coronary intervention on patients with acute coronary syndrome in Iran: A prospective observational study
Background: Frequent Percutaneous Coronary Intervention (PCI) procedures are being performed on a daily basis in Iran. However, no study has been reported on the current PCI practice in patients with acute coronary syndrome (ACS) in Iran. We aimed to describe the clinical characteristics and treatment patterns in Iranian ACS patients treated with PCI. Methods: Between February 2017 and July...
متن کاملSuccessful management of patients with a drug-eluting coronary stent presenting for elective, non-cardiac surgery.
This paper describes the management of three patients for elective surgery with drug-eluting stents in the coronary circulation. The risks posed at the time of surgery by such patients include acute coronary syndromes, as a result of stent thrombosis, after cessation of anti-platelet therapy and excessive bleeding from continued anti-platelet therapy. We describe a regime for the management of ...
